Community Health Case Management
Overview

Our Case Management Team is here to connect you and your family with resources to help during challenging times and support your goals for the future.

NMCS’ Case Management program is critical in connecting individuals and families to essential services, resources and programs to meet your basic needs. Our Case Management staff want to understand your needs and strengths to support you and/or your family so that you may confidently move forward with your own goals. These resources can include: emergency rental/housing assistance, food pantry or CalFresh enrollment assistance, linkages to public benefits assistance, referrals to other community programs/resources, access to childcare and healthcare services, employment support, and more.

As an organization, NMCS strives to connect individuals and families to essential resources and services to address economic, housing and food insecurities. Upon inquiry with NMCS staff, an individual is pre-screened and a comprehensive assessment is completed to assist in determining the goals of the individual or family. While meeting with a Case Manager, participants have the opportunity to articulate, prioritize, and plan their needs and goals with the support of Case Management Staff. Case Managers will help identify and link participants to the appropriate services and resources while advocating for the individual or family with schools, healthcare providers, and other community service organizations.

Case Managers will offer support and continuous contact to work with families as they take steps toward greater self-sufficiency.
